Kindergarten Registration

When you take your child to register him or her for Kindergarten, the documentation that you must bring with you includes:

• An official birth certificate
• Proof of address which may be any two (2) of the following:
  • Parent/Guardian driver’s license or CT State ID
  • Current Utility Bills from two different companies
  • Canceled checks to prove payment of rent for (2) months
  • A notarized letter from landlord stating family lives at that address
• Immunization Record/Health Assessment Record with BOTH sides completed
